[rt-users] Adding custom html entries

I can't really explain this, but I'll try to do the best I can do...

I have this webpage (you can view it here http://www.callatg.com/~pburkett/atg/config_request.html)

What I need to do is make a template like that, so when a user fills in a new trouble ticket, it will resemble this. I tried checking out the RT documentation but somehow it wont all load on my browsers (Konqueror and Opera.) Any help would be appreciated thanks.
