[rt-users] Approvals Not Working

Using RT 3.0.9

Approval documentation is not clear on how to set this up.  I have tried the examples from the Doc but they are not working.  Here is my log output...

Action: Create Tickets 
Stage: TransactionCreate 
Template: Operations Approval
