You'll want to turn on the MySQL slow query log to find what SQL queries
are taking overly long -- see

instructions, but they mostly boil down to setting

Providing `SHOW INDEXES FROM Transactions` and `SHOW INDEXES FROM
Attachments` may also be useful.
