I haven't looked into where it comes from, however our installation of RT3
automatically shows a favicon of a black box with a red checkmark through
it.

Aha, in the Header template I see:

<link rel="shortcut icon" href="<%$RT::WebImagesURL%>/favicon.png"
type="image/png">

So there you go.  You can find RT's official favicon in your images
directory.
