I've found the same thing, failure at initdb.cgi
I've communicated directly with Autrijus regarding some of the issues.
Here's a summary:
Start With Win2k server, sp4 running on a Vmware virtual machine.
.NET installed (autrijus says he is using a .net box for devel)
After installation I ran into the problem where the browser can't find
initdb.cgi
Attempting to manually start apache shows that it can't find the correct
document root, looks like it never inserted the path during install into
httpd.conf
This prevents me from starting apache to perform a reinstall while the
service is running. I've modified httpd.conf to reflect the correct
path to the rt document root, which allows apache to start.
Unfortunately, after I get to that point reinstallation doesn't seem to
make the neccesary modifications and I still can't get initdb.cgi to
run.
It appears that the httpd.conf in the apache server root is missing a
few load module lines. When I add them manually I am unable to start
apache due to issues with loading mod_perl.so
I'm continuing to work on this, and I'll start by following your
apachetriad method and let you know how it goes.

I've been suffering as well. The installer failed at the initdb.cgi.
However I've now made progress! It seems there is a chicken and egg
issue - the Apache RT service must be running before the database
initialisation starts. I actually loaded the appachetriad build of
apache, perl, php and mysql. I think this meant that I had an apache
server running when I next tried the rt installer. Unfortunatley I'd
found the initialisation for the database schema and created an rt3
database manualy. This then throws out the initdb.cgi - it has a branch
which creates everything if there is no rt3 database already. But,
significantly I now had the Apache RT service running. With the rt3
database removed and the RT service restarted the initdb.cgi then
worked. And that's as far as I've got. Hope that helps....
