You did not run the database upgrades between 3.6 and 4.0

Can you tell us what you did do?  

You need to review all of docs/UPGRADING-3.6 3.8 and 4.0 and the README.
It appears that you've skipped all of the 
rt-setup-database --action upgrade --dba root 
steps and the standalone scripts.
