[rt-users] Aprovals

Im having some trouble with aprovals and so far google hates me and returns nothing when i search.  i have setup the script:
On Create Create tickets with template Aproval

here is the template aproval:

Queue: Approvals
Requestor: {$TOP->Requestor}
Type: Approval
Owner: kpietri
Depended-On-By: {$Tickets{"TOP"}->Id}
Refers-To: {$Tickets{"TOP"}->Id}
Subject: Approval for ticket: {$Tickets{"TOP"}->Id} - {$Tickets{"TOP"}->Subject}
Content-Type: text/plain
Content: Your approval is requested for the ticket {$Tickets{"TOP"}->Id}: {$Tickets{"TOP"}->Subject}

here is the relevant lines in the log file:

there is no aproval ticket being created.  Any help would be greatly apreciated.

ps.  on a side note im getting this strange chunk of code in my log files (its alot so i put it on our web server) http://ww2.nvps.net/~karl/rtlog.txt  not really my concern at the moment and is totaly un related.
