Please try to change line from
$RT::Handle->ForceRollback() if $RT::Handle->TransactionDepth;
to
$RT::Handle->ForceRollback() if $RT::Handle && 
$RT::Handle->TransactionDepth;
and see if it gives other error, more informative.

Looks like you can't connect to your DB.
Turn on debug RT log(http://wiki.bestpractical.com/?Debug)
and see if it has some records.
