Sorry about the "Advanced" format shot. Here it is:

'<b>__id__</a></b>/TITLE:Ticket Id', QueueName, 
'<b>__Subject__</a></b>/TITLE:Subject','__Requestors__', Priority, 
Status,'__CustomField.{Work-State}__', OwnerName, 
'__Due__</a></b>/TITLE:Due Date'.
